An easy-to-print Smile Designs Prescription Form in PDF format is available here.
Delivery: Guelph and area is serviced by our own driver.
Our out of town accounts are asked to use I.C.S. courier or Purolator.
Call us and we would be pleased to make arrangements for the courier to pickup at your office.
